508 Unusual Incident on Board Premises

The Tri-County Board will review the circumstances of any event that poses a danger to the health and safety of Board members, staff and visitors.
An “incident” is defined as any event that poses a danger to the health and safety of visitors, Board members and/or staff, and is not consistent with the routine operation of the Board. Incidents shall include all deaths, serious bodily injuries, alleged criminal acts, alleged abuse or neglect and any life threatening situation. This policy is applicable to incidents taking place on Board premises, regardless of who is involved and with incidents involving Board staff or Board members while serving in their official capacity regardless of the location.

The Safety Officer will maintain a log of all incidents and shall annually submit a summary report to the Executive Director. The Executive Director will inform the Board of Directors of any need to change policy or any corrective actions as a result of an incident as needed.

Board staff shall be required to:
- Promptly report all incidents to the Safety Officer
- Prepare a written report which identifies the persons involved and which describes the essential facts of the incident and how it was dealt with
- Notify law enforcement, emergency personnel or other public officials as appropriate
- The Safety Officer will review all incidents to determine what corrective action may be warranted. This review and follow up process shall be documented.
- Incident reports should be reported within twenty-four hours after the event.
- Board employees may be required to complete a worker compensation form and other applicable Miami County forms for injuries.
