418 Reporting Changes in Personal Information

Failure to report changes in personal information may prevent employees from obtaining or maintaining valuable employee benefits or services. It is each employee's responsibility to report any change of personal information within five (5) working days of the occurrence of the change. Notification shall be made in writing to their supervisor.
For the purposes of this section, a change in personal information shall include the following:
Employees shall report changes in personal information to their immediate supervisor within five (5) working days of such change.
Supervisors will make certain that notification of any change is immediately forwarded to the Director of Finance and Administration, the Personnel/Benefits Office, and the County Auditor's Office.

For the purposes of this section, a change in personal information shall include the following:
- Name change;
- Address change;
- Phone number change;
- Marital status change;
- Changes which may affect employee benefits (i.e. changes in dependents or beneficiaries).
- Number of exemptions for tax purposes
- Citizenship;
- Selective service classification; or
- Association with a government military service organization.
- Changes with emergency contacts
Employees shall report changes in personal information to their immediate supervisor within five (5) working days of such change.
Supervisors will make certain that notification of any change is immediately forwarded to the Director of Finance and Administration, the Personnel/Benefits Office, and the County Auditor's Office.
